If you can get X to recognize the buttons, it should work just fine. I have a mouse with two thumb buttons that didn't do anything in RO before, but I tried assigning "9" and "0" to them and they work in game.

I have the Razor Naga and I run Linux and Windows. Pretty much the only thing I can get the buttons to do on my Linux side is cast spells. 1-10 cast the spells on your current bar and that can be really handy.
On my Windows side I have it set up so that I can press one button and all my buffs go off. You can set the macros to set off any number of spells. It is amazing just being able to press one button and everything casts. There might be a Linux program to download, but I am not sure. Heru uses this mouse and he was trying to explain it to me once, maybe talk to him.

Mine were recognized as mouse buttons 8 and 9, but I'm guessing it doesn't make a difference either way.

Without being programmed on windows first? Interesting, I thought those buttons just send nothing without software...

I use a software named "btnx" with a razer mouse (unfortunately not 12 buttons) and Ubuntu 11.04. It was easy to install and configure, and works fine for me.
