yes. this is difficult question. What DO we do with items which are in "inactive" accounts? Do we still include them in the probability distribution of items?
Personally, the cleanest solution in my way of thinking would be to have 1) everyone gear previouslky selected from an armoury identical across all realms 2) you can get time limited boss items, say limited to 2 weeks to 3 months or so. then the probability distribution of gear on every server instance should be relatively easily calculated.
Under the system prevailing across the mmo industry, you have massive problems with perpetuating imbalances because players end up paying real money for permanent items, which acts to permanently imbalance pvp over time. it seems to be poorly thought out and ill-suited for pvp mmos.
