Fatigue adds up over time.

For example, if you played 3 hours and 30 minutes without fatigue, then logged out, then logged in again a while later and killed 5 mobs, then 30 minutes later you'll have fatigue.

Also, you're supposed to email ngd about fatigue reset failures.
